Financial Agreement and Consent

Self-Paying Patients

I agree to pay for all healthcare services provided by Enigma Healthcare in accordance with the clinic’s terms and conditions. I understand that payment is due at the time of service unless otherwise agreed in writing.

I acknowledge that my personal data may be processed as outlined in Enigma Healthcare’s Privacy Policy for the purposes of my treatment or care.

I consent to the disclosure of my patient information in accordance with the terms set out in the Privacy Policy.

I understand that any outstanding balances on my account may be referred to third-party collection agencies for recovery if not settled within the agreed timeframe.

Insured Patients

In cases where a third-party insurer agrees to cover part or all of my medical fees, I accept responsibility for any remaining balance not covered by the insurer.

I confirm that the specialist treatment I am receiving has been recommended by my general practitioner and that the information I have provided is accurate and complete.

I authorise Enigma Healthcare to submit claims to my insurer for treatment provided, and I understand that I may be required to assist in the claims process if needed.

Terms of Admission for Patients

Enigma Healthcare is registered to provide healthcare services in partnership with independent medical practitioners.

All consultants working at Enigma Healthcare are self-employed and have been granted Practicing Privileges by the clinic. Each consultant is responsible for their own professional conduct and actions. Consultants are required to comply with Enigma Healthcare’s Practicing Privileges policy, which includes maintaining appropriate medical malpractice insurance.

Charges for services provided by consultants with Practicing Privileges are included in Enigma Healthcare’s overall billing structure.

Enigma Healthcare operates a complaints policy, which is available for review at the clinic’s reception desk upon request.
